In preparation for a patient’s first visit, we like to offer the opportunity for the patient to preview all of the information we’ll need to start the patient’s file. We ask that all patients come to their first visit with the following information prepared:
- NPP Acknowledgement & Patient Communication Consent Form
- Financial Policy Form
- Health Information & Record Release Authorization
- New Patient & Other Forms Overview
- Insurance information and insurance cards
- At least one (1) form of identification (drivers license, social security card, birth certificate, etc…)
